In organising the sale of my parents land I was advised that I had to lodge a vendor duty form and was directed to the Office of State Revenue website.
(From 1 June 2004, vendors or transferors of land-related property in NSW are required to pay a vendor duty of 2.25% of the dutiable value of the property. The vendor or transferor is liable for vendor duty.)
There are heaps of forms available and all for download here -

i.e. Application for 1st Home Buyers Grant & various Vendor Duty forms just to name a few.
