'on plan of subdivision'

In regards to a Victorian contract note, what does this mean exactly?
subdividable or already been subdivided?

'lot x on plan of subdivision'
It's just a reference to the lot in question. The plan of subdivision may be 50 or 100 years old, but pretty much all lots are a result of a subdivision at some time in the past. The "name" of the lot then becomes exactly as you say "Lot xx on Registered Plan xxx".

The name alone doesn't really tell you anything about whether the lot has been "further" subdivided. If you were buying a lot that had been split into two, though, then you'd usually be buying two lots, eg "Lot 1 and Lot 2 on RP xxx". Look at the plan, and it will show you what each lot is and what its boundaries are, which will tell you exactly what it is that you're buying.
