For those who are interested, the ATO now has the 2006/2007 Income Tax Withholding Variation Application form on their web site.
It can be found here: http://www.ato.gov.au/businesses/content.asp?doc=/content/6650.htm
I couldn't get it to work using Mozilla Firefox but it worked fine with IE.
When you submit the form it says "It may take 10 minutes to send" and it does so be patient
.
