Are there any penalties or costs if a tenant decides to break a lease and vacate a property early?

What is the minimum notice they must give?

If tenants decide to break a fixed term lease they will be liable for all costs for finding new tenants.

This would include payment of additional rent until new tenants move in, final bond inspection costs, proportion of the new tenants letting fee.

In short you shouldnt be out of pocket. For periodic tenancy they can give a period of notice to vacate (~60 days from memory).

Tenant has to give 21 days notice if they wish to vacate a property on a preiodic lease (as I recently found out)

Landlord has to give tenant 60 days notice - during these 60 days tenant can give 2 weeks notice that they are leaving
