buying -title not tfrd from previous owner mortgage issues

my sister is interested in this house however it appears that the title was not transferred from the first owner to the current owner. i dont know how this can happen? there is already a mortgage on this house under the current owners. can someone please let me know if this is common? the contract of sale doesnt state the current owners name as well.
