Help with ITWV

Hey guys, Just got a few questions about filling in the ITWV form..

*With the section "Rental Details" the Percentage Owned question, are they asking how much you own compared to the bank ? or as in how many people are title holders ?
My names the only one on the property so dose that mean i put 100% ?

*I only have 1 IP and no other investments at all, so for Section D "Investment Details" do i just leave all of the blank ?

*Section E4 about superannuation, I dont understand any of that.. Where would I find the information for that section ? on my payslip ?

*Section E16 Medicare, i also have no idea about this section aswell.

Yes - 100% if you're the sole title holder.
Just leave D blank if you have no other investments.
E4 relates to payments FROM super, Termination Payments etc etc. If you don't have any of these, leave it blank.
If you're not claiming exeomtion from the Medicare Levy due to any of the reasons given, then leave that blank, too.
